McMINNVILLE, Ore. – An Amity woman died after a head-on crash along Highway 99W south of McMinnville Monday, police said.

Investigators said a Ford Flex driven by Terrence Chitwood, 67, of Amity, caused the multi-vehicle collision. The Flex was traveling southbound on Highway 99W, when for unknown reasons, Chitwood crossed the center line and crashed into an oncoming Toyota Yaris.

The driver of the Yaris, identified as 65-year-old Kristyanne Devries, suffered critical injuries. She later died at Oregon Health and Science University, according to Oregon State Police.

A third vehicle, a Subaru Forester, was traveling northbound behind the Toyota and also collided with the Flex.

Chitwood and his passenger, Doris Chitwood, 66, were taken to Willamette Valley Medical Center for treatment of minor injuries.

The driver of the Subaru, Michael Miller, 37 and his passenger, Lindsey Patterson, 34, were taken to Salem Hospital with minor injuries. They are both from Corvallis, OSP said.

Highway 99W was closed for over three hours during the investigation and as crews worked to clear the wreckage.
