PORTLAND, Ore. (AP) -- Oregon officials have been laying much of the blame for the state's health insurance exchange troubles on its technology contractor, Oracle Corp.

But a review by The Associated Press found that much of the blame also lies with state officials' own actions.

Health officials refused to back down from an ambitious vision even when there were signs it was flailing. They disregarded advice and decided to do complex software work on their own. And they botched the handoff of the work from one agency to another.

Many of the mistakes were not made by the exchange itself, but by the Oregon Health Authority, which controlled the money and performed most of the technology work.
